I have scheduled three away rotations, all confirmed, but now, I think I want to cancel the last one and do it at another school. Will doing this screw up my chances at the school I'm cancelling?

Right now, I think I might get an interview at the school I've scheduled without doing an externship, but I want to schedule a school that I have little chance of getting an interview without spending a month there.

If you cancel a month in advance, it should be ok. Just don't tell them that you are cancelling to go to another program. Odds are with the way that paperwork gets lost in the shuffle, you'll still end up getting an interview at the original place anyway.

What I have heard jives with what IrishDoc said. Give them enough lead time, and notify them in writing.

I'm doing an Ortho rotation now at my home school. Just don't do what some kid did and schedule the rotation and not show up or not tell the program he wasn't coming. The kid was put on a busy team, and we had incorporated him into the call schedule. He pretty much made a lot of people's lives harder. I'd say he has no chance of getting into this program since the residency director and about half the residents now know about this.
