Extra application requirements

Does anyone have a list of programs that have extra requirements outside of ERAS like a form or request for a programs specific shoutout in your PS??

I know it is on their webpages, but it would just take forever to look at over 100 of them so maybe we could start a list if it doesn't already exist.

Here is what I have so far:

Brown: Eval form for your letter writers
Yale: Eval form from letter writers
NYU: Tell us why you want to come to NYU in your PS

Anybody have others you've encountered?
WashU - LoR supplement
UW - Supplemental form from 3 surgeons
UVa - Supplemental app

Vanderbilt: applicant evaluation form, and you have to first click "how to apply" on the right side of the page then click "useful information" which only shows up after you click the first link; having to submit those forms is not listed anywhere on the main "how to apply" page.

Just feeling a little annoyed since I'd already provided my LoR writers with multiple other forms and I feel badly to have to contact them yet again with another one.

University of Oklahoma - OU personal statement - ties to Oklahoma/surrounding region and/or explanation of why you believe you would do well living and training in Oklahoma for five years. Your class standing if you school ranks students.

Oh and I think that assuming no one is applying ortho would be wishful thinking... but all I've heard is that it's a pretty big year at a lot of programs.

I know Harvard does, but there have got to be others for sure...
