UConn fits all those criteria. I rotated there in November which was too late to get a letter but they have some pretty big names. The rotation itself is pretty self-directed so you can spend as much time as you want with whichever faculty you like. The residents are great and the faculty is definitely not malignant.

i think that mt sinai also fits the bill. they heavily favor rotators, the chairman is nationally known, there is a very good balance of all specialties (although the trauma rotation is at queens), and for lifestyle, it is very bottom heavy, ie, busy during PGY-2,-3, and light during PGY-4,-5 (so it averages out to *reasonable* i think)

I'm going to throw my 2 cents in about Mt Sinai.

In addition to Springfield (tumor), they also have Flatow (shoulder prosthesis named after him) and a bunch of other well known guys. They recently added a few new surgeons, and I heard some of them came from Lenox Hill (I'm not sure what the story is there, but I heard Sinai came out with some more good guys in the end). Their main hospital is Level 3 trauma, so nothing crazy while on call there, but you get good exposure at their Level 1 in Queens when you rotate over there. Lots of good floor help in terms of PAs and nurses, and you operate early and often. This place could be tops on my list when the time comes.

Doing a rotation there has given you a good advantage in the past.
