procedures with drastic improvement for the patient?

What orthopedic procedures lead to the most drastic improvement (in terms of overall functionality, pain, etc) for the patient? Yeah I know this is a vague question, but I'm just curious since I've mostly been exposed to knee and total hip replacements as well as some sports medicine stuff in school so far.
THRA & Resurfacing.

I find intramedullary nailing of femur and tibia fractures to be high yield and fun cases.
I an hour with minimal incisions and soft tissue destruction you can do some good for the patient.

+1 for intramedullary nailing. When you are on the wrong end of traction pins, knowing that you won't need to stay in bed for 2 months without moving is quite nice. I can't imagine sitting in traction or being in a spica cast.
