Thought it may be a good idea to start a thread for programs that require something outside of ERAS and 3 letters of req.

Please post as you come across in your searching

1. Case Western - Official Undergraduate Transcript
Some clarification on Case. You don’t have to have an official transcript sent from your undergrad registrar’s office. You can email or fax a copy that you have in your possession.

2) Drexel: SAT and certified MCAT scores

3) University of Washington: 3 Supplemental evaluation forms

4) Washington University: 3 Supplemental evaluation forms

5) Yale: 3 Supplemental evaluation forms

6) UCSD: Email SAT scores; official undergrad transcript

7) UMDNJ-Newark: MCAT scores

NYU: personal statement about why you want to attend NYU

9) Tufts: Non-ortho letter

10) UCSF: Non-surgery letter

I thought UCSD was MCAT not SAT?

Additional info:

UCLA: MCAT
UC Irvine: ACLS/CPR training
UCLA Harbor: MCAT
USF: Undergrad transcipt on request
New Mex: Mailed letter of interest
U of OK: Speific requirements for PS or mail/email additioanl info
Wichita: Hand written PS
Texas Tech: Supplemental form for standardized test info

Vanderbilt has a supplemtal form like Yale, Wash U and U Wash.

Oh and...

Once you apply, UVA sends you an email with a form requesting additional info, it almost like a second PS.

Vanderbilt has a supplemtal form like Yale, Wash U and U Wash.


I heard that the supplemental forms for these 4 programs - Vanderbilt, Yale, Wash U, and U Wash - weren't required for your application to be complete. Does anyone else know or has heard anything about these forms being used as additional helpful information, but not necessarily required?

I should clarify my last statement regarding the supplemental eval forms. For example, if you submitted 4 letters of rec to one of these programs, and only 2 of your letter writers completed the supplemental form, would your application be considered incomplete or would they still review your app with only 2/4 of the supplemental forms? I had heard that you don't need all of your supplemental forms in order for your application to be complete, but does anyone with more concrete knowledge know anything about this?

Of those four, the only one that specifically states that it is required for the application to be complete is Univ of Wash.

I emailed the administrative assistant at Yale:
I am currently applying to the orthopaedic residency program at Yale. I have a question regarding the supplementary letter of reference form. Will applications be considered if I am unable to have my letter writes submit this form. I worked with two of my letter writers at an away rotation and would have a hard time getting them to submit this form.

Her response:

We will still consider your application...not a problem.

I actually went to the webpages of all of the programs to which I applied and usually they're pretty explicit about what they do/n't want. It took me all of about an hour to make an Excel table. You're paying $30 a pop, so you might as well make it count.

Some places have different reqs just to see if a) you read the webpage and/or 2) to see if you're committed (um, Wichita). With the app:spot ratio of ~100:1 for most places, it wouldn't take much to turn away an applicant, so it might be in one's best interest to do what they ask for, no more, and no less.

If in doubt, email the program contact and make a friend.

how strict do you think indiana is about the october 1 deadline? i got everything in but a recommendation. one of my recommenders has been a little slow going. thanks.

Well, I didn't apply to Indiana because I knew I wouldn't have my required stuff ready to go by then.
