programs that require extra personal statement or forms?

I know there are a few programs that ask for an extra personal statement, something you submit outside of ERAS. Anybody have a good list of those programs?
NYU requires a personal statement that says why you want to go there

Thanks, I've also found that Washington State requires an extra form be filled out by your letter writers.
