Can those of you with info. about any programs that require more than three letters of recommendation, or specify a particular letter come from a chief etc. please post that info. Also, any other specifics that differ from the norm.

It will save us all a lot of web-surfing if we work together.

Lets see...

1. UCSD - undergrad transcript as 4th LOR
also MCAT scores at end of PS.
2. UCSF - one non-surg LOR, copy of AOA thing (don't know what thats supposed to be)
3. Tufts - 1 non ortho LOR
4. NYU - a whole buncha crap for your personal statement: gotta recheck the website for this one.
5. Hawaii - 4 LOR (one chief)
6. Harvard - 4 letters (one chief)
7. SF residency program: 4 LOR (don't know about chief letter yet)
8. LA Harbor: MCAT scores


Thats what I got so far. Also, I heard LA Harbors not accepting applications this year. Anyone know if there is any truth to this?

1) Yale - requires supplemental forms from evaluators
2) University of Florida - requires chairman's letter
3) Tulane - requires chairman's letter
4) Kalamazoo - requires completion of applicant questionnaire
5) University of Michigan - requires chairman's letter
6) University of Rochester - requires chairman's letter and supplemental form
7) SUNY Buffalo - requires chairman's letter

In addition to the programs listed above, I have info for the following places:

1. U of Illinois College of Med at Chicago- chairman's letter
2. Southern Illinois U- chairman's letter
3. Union Memorial- chairman's letter; all residents 6 yrs.
4. U of Massachusetts- must specify on application if interested in surgical intern yr (program begins in PGY2 yr)
5. Johns Hopkins- 4 letters
6. UNC- SAT/MCAT scores, reprints of published articles
7. UMDNJ New Jersey Med School- college transcripts, MCAT, supplemental form
8. U of Iowa- college transcripts
9. Seton Hall- 4 letters, one must be from chairman
10. Ohio State U- chairman's letter
11. Penn- college transcripts
12. Drexel- only 2 letters, SAT/MCAT scores
13. Allegheny General- 4 letters
14. Virginia Commonwealth- only 2 letters, personal statement must be autobiography discussing career goals/outside interests

In regards to U of Rochester, I saw the mention of a supplemental form in the past, but now I can't seem to find any sign of it on the website. Have the requirement changed, or am I just missing something?

1) handwritten PS for UK-Wichita
2) kindergarten art projects and the 3rd grade "What I did last summer" essay for Grenada.
