I am a US citizen attending a carib. school and would like to apply for an orthopedic residency. I am currently studying for step 1 and understand that I need an excellent score. My question is as follows:

If I do not do as well as needed on step 1 or step 2 but I am able to get into a surgery rotation what can I do to switch into an ortho residency after my first year? And will I still have to do five years or only 4?

Also if I simply complete a surgery rotation will I be able to do a fellowship in something such as Hand surgery or joint reconstruction (hip/knee replacements) as a surgeon?

As a side note I am from the Tristate area (Northeast) and do not know if I am going to be able to do a residency elsewhere for reasons beyond my control (family). Are NY, NJ, CT hospitals IMG friendly in this field?

I find the field of orthopedics to be most interesting and I am looking forward to doing my rotations through ortho. I have read it is a good idea to do aprox. 3 of them in order to show my interest and hope to do well enough to get accepted. However if I am not do I have the options I ask about above?
Does anyone know about this?

Traditionally, there have been three routes to get into hand surgery: orthopedics, plastics, and g-surg. The first two count for most of the cases. So, you may be able to score a hand fellowship after g-surg.

As for adult reconstruction, I have never heard of anyone doing one without doing an orthopedic residency first. When you would go in practice, you would need to be board certified in orthopedics since there is no subspecialty certification in adult reconstruction and that requires having completed an accredited orthopedic residency.

Thanks for the reply. I am going to shoot for an ortho residency but wanted to know what options I have as a back-up.

One other problem I have is research. I have no research up to this point and honestly I am not sure how to get involved in any since I am going to be traveling throughout my rotations (I will probably not be in once city/state for more than 3 months.

Can you (or anyone else) offer me any advice on where I should look for research opportunities that I can be involved in while traveling?
