How do "2nd look" visits usually work? Do you tag along for a half day? all day? go to the OR? And what do you wear - business casual?

You contact the program(s) individually and work out how it can be done. Some programs want you to stay for a few days, srub in, meet the director/chairman again etc. Some may want you to stay for just a day or 1/2 day.

I wouldn't bother with second looks unless it's a local program for you. A friend of mine did that in an early match specialty and it didn't help him at all- just cost him a lot of extra money. Just give your top programs a call, send some letters, or better yet, have your program director or chairman call for you. If your PD calls, he will usually find out how you will be ranked and you may be moved up if they know you want to come there.

Call me paranoid....
How do PD's feel when you tell them that you really want to go to a program other than your own? Seems like it could come back to haunt you.
