Well, found out about UNC, don't need official forms, just self report it.

Any report on these programs, plus UT-Houston and Yale?

There are some good old posts on this topic...I think it's called "extra info" or something like that. it tells you what programs need what extra info.
As for paper CV, some places specifically request it, some don't. One option is to scan it in as a 4th LOR and send it to all programs (except the ones that require 4 LORs), then you don't have to find out every single program that wants one. Also it will be a nicer copy and compilation of your honors/talents/etc that you can organize in ways that show off your strengths.

Also, beware of programs with reputation for only taking rotators. Georgetown is one of them.

Baylor and San Antonio are both in somewhat of a disarray (at least last year when I matched). Both looking for new chairmen, both known as malignant. However, they both have tons of OR experience and autonomy.

If you are looking in Texas, look harder at UTSW which is most circles considered the best program in Texas. Very stable faculty that are excellent, good OR experience, but will work you very hard as a resident, and weak in spine & sports. UTSW places lots of value on doing rotation, but beware that residents basically have the power to pick the incoming class. If you don't get along with the second year you are assigned you won't get in.

As an ortho intern in the south who rotated at UTSA and interviewed at Baylor last year I feel that I know both programs fairly well. First Baylor, it is somewhat malignant from the info I got from their students on the interview. Overall though it is a well rounded program, they do get a lot of operative experience. I am not sure what is going on with their chairman situation, I could not get a straight answer. UTSA was totally unimpressive. Although I rotated on Rockwood's service, I saw the man only once in two weeks. The residents do operate a lot, but not until they are upper level residents. On almost every case a chief and a junior are scrubbed, not to mention the medical student and attending, so take it from there. Also, the faculty are not necessarily resident advocates and they tend to lose their temper quite often (to put it mildly). I have heard through the grapevine that they are close to probation. I ranked both schools, but I think I am much happier at my program than I would have been at either of those two programs.
