Ive got a 235 on step1. I'm trying to figure out where to do aways. Would it be wise to shoot for the top (ie Stanford, Rush, Columbia) and try to get great letters from these big name programs or stay in the middle tier that would be more reasonable due to my step1 score? Any help choosing aways would be much appreciated.

I assume this means you're not in the current application cycle. Why not think about doing one at a big name place and bank on getting a letter from a big name person there whose name alone might pull some weight in the application process? 235 is still probably good enough to get you through most programs' filters, so having a recognizable person say something good about you might go far. Then rotate at a "less competitive" program(s) too (yeah I know they're all competitive...). I know people who did about the same as me in school but had letters from big names and got more/better interviews out of it (especially regionally based interviews - like rotate in NYC if you want to increase your chances of getting interviews in NYC). Make sure you do well 3rd year, take Step 2 early and try to improve.
