It's time to pick rotation order at my school. I've heard a lot of mixed advice. We don't get any electives (no ortho), so my most important grade will be in Surgery. When is the best time to take it to honor it?

I've heard the following about scheduling GS:

"Take it after medicine and OB, they will help with the shelf and you will know your way around the OR."

"Don't take it first, you won't know much about clinical medicine and the shelf is hard."

"Don't take it in the middle of the year, all the gunners will be in your rotation and it will be much harder to shine."

"Take it first, they won't expect much and you will stand head and shoulders above those who are taking it first just to get it over with."

Does anyone have any advice or opinions regarding the mixed advice I've heard? And how much harder is the shelf if you haven't taken medicine first? (At my school, surgery and medicine are always in different semesters.)

Gen surg was my first rotation of third year and I honored it. Part of the reason why was because the shelf exam means were so low. The longer you wait, the higher you will have to score to get to the top 5% (as it is in my school).

Medicine was immediately next....smacked that upside the head...because I didnt have to do the shelf exam at the end of the year when everyone's a pro.

I've been told to take it after IM and OB/Gyn, but taking it earlier does make sense to me as well.

In my class a few years back, 9 out of 37 people who took Surgery first FAILED the shelf. That's 25%. This was at a top-5 med school.

Just some food for thought...

I agree with previous posts that the ideal placement for 3rd year Surgery rotation is probably around the 3rd or 4th out of 6. I don't think you should start with Medicine though because that is a tough one also. Maybe go Ob gyn, IM, then Surgery. So you are comfortable with the NBME's and OSCE's by the time you get to Surgery. Good Luck!
