At a minimum, you should give your letter writers the following:
1) CV - include your USLME Step 1 score and any pre-clinical and clinical honors so they know how you have performed prior to your month
2) Personal statement - as we've discussed previously please proof-read it (and have someone you trust critique it before you submit it)
You can also include transcript, list of programs to which you are applying and any reprints of publications if applicable.
The ERAS application includes everything (CV, application, letters of recommendation, personal statement, etc...). Your personal statement can cover anything you like but remember no more than 1 page and don't use anything smaller than 11 font.
