So I have an interview at yale next week on the 14th - was just browsing their website and found out that they had requested that "a Referee evaluation form for each letter of recommendation written is requested" - basically it is one of those supplemental application forms to be filled out by the people writing letters of rec. I was offered an interview without ever getting these forms filled out.

I just became aware of their existence like 5 mins ago - so should I try to get it submitted late (may not get it turned in at all by the time of interview)? Or else how do I explain the fact that I never got it turned in? I think I'm going to look pretty stupid when they are looking through my file and see that I don't have any of those forms filled out by my evaluators.

Had same experience with Wash U (was offered interview without getting their supplemental form filled out) So unsure of forms importance. I'm guessing that I should try and get the forms filled out -what do you guys think?

Any advice appreciated. Thanks.

Had my interview on 11/30 and like you did not fill out the forms and was pretty much not expecting an interview but I did recieve one.

Good news!!!! None of my 4 interviewers mentioned it so dont sweat it. If you can get it do..if not do not let that deter you. They have a great program.
