Thought it was time to start a 2004-05 edition of programs which require supplemental materials outside of ERAS or have special requirements.

Here's my list so far (info from program websites and FRIEDA; please correct any inaccuracies):

-U of Iowa = college transcript
-U of Michigan = chairman's letter
-Kalamazoo = applicant questionnaire from their website
-Mayo = college transcript
-Wash U = supplemental LOR forms
-UNC = chairman's letter, MCAT & SAT appended to PS, copies of pubs
-UMDNJ (New Brunswick) = college transcript
-U of Cincinnati = chairman's letter
-Case Western = college transcript
-Ohio state = chairman's letter
-Vanderbilt = supplemental LOR forms
-Med college VA = short autobio w/career goals & outside interests

Plus....

Southern Illinois = 1) deans letter with senior year curriculum, and 2) Letter from Chairman, Dept of Ortho
UMKC = their standard cover sheet with each LOR
Wichita = hand-written statement

I tried to do a search of all the old posts on this and assemble them in to a list (probably with lots of repeats, since I havent looked through it completely).

1 U of Illinois College of Med at Chicago- chairman's letter
2. Southern Illinois U- chairman's letter
3. Union Memorial- chairman's letter; all residents 6 yrs.
4. U of Massachusetts- must specify on application if interested in surgical intern yr (program begins in PGY2 yr)
5. Johns Hopkins- 4 letters
6. UNC- SAT/MCAT scores, reprints of published articles, chair letter
7. UMDNJ New Jersey Med School- college transcripts, MCAT, supplemental form
8. Iowa - college transcripts, MCAT, SAT
9. Seton Hall- 4 letters, one must be from chairman
10. Ohio State U- chairman's letter
11. Penn- college transcripts
12. Drexel- only 2 letters, SAT/MCAT scores
13. Allegheny General- 4 letters
14. Virginia Commonwealth- only 2 letters, personal statement must be autobiography discussing career goals/outside interests
15 UK-Wichita - handwritten PS
16 Yale - requires supplemental forms from evaluators
17 University of Florida - requires chairman's letter
18 Tulane - requires chairman's letter
19 Kalamazoo - requires completion of applicant questionnaire
20 University of Michigan - requires chairman's letter
21 University of Rochester - requires chairman's letter and supplemental form
22 SUNY Buffalo - requires chairman's letter
Duke - "requires" chairman's letter
23 Vanderbilt - requires supplemental forms from evaluators
24 Washington University - requires supplemental forms from evaluators
25 University of Washington - requires supplemental forms from evaluators
26 UCSD - undergrad transcript as 4th LOR also MCAT scores at end of PS. 6year program
27 UCSF - one non-surg LOR, copy of AOA thing (don't know what thats supposed to be)
28 Tufts - 1 non ortho LOR
29 NYU - a whole buncha crap for your personal statement: gotta recheck the website for this one.
30 Hawaii - 4 LOR (one chief)
31 Harvard - 4 letters (one chief)
32 SF residency program: 4 LOR (don't know about chief letter yet)
33 LA Harbor: MCAT scores
34 Johns Hopkins - also wants 4 LORs
35 Univ. of Michigan - has a "voluntary ID form" to fill out on their program website
36 NYU/HJD - in your personal statement, they want to know why you like HJD specifically
37 MCP/Hannemann - SAT score, MCAT score
38 Brown University - this is actually a 6 yr program (5 yr residency and 1 year of extra junior attending mandatory but you will be awarded a trauma fellowship according to website)
39 UMKC - Extra form for LOR
40 Okla - Add to PS why you would like Oklahoma Region
41 Baylor - Extra form for LOR
42 UCLA Med Center -- MCAT written in PS
43 Kalamazoo -- Questionaire scanned & attached to PS
44 Mayo -- Undergrad & Grad Tx, CV
45 Akron Gen Med -- Med stdnt performance eval (what is this? the residency coordinator never replied to my e-mail)
46 Cleveland Clinic -- Cleveland Clinic Application Form
47 Summa -- CV
47 U Pitt -- CV

I've been coming up with the same results from websites, FREIDA, etc. Regarding the supplemental LOR forms - at least five of the programs I'm applying to require them. Does anybody know how strict they are about having a form returned by EVERY letter writer? I'm just trying to figure out the best way to ask attendings for letters, and then say, "Hey, would you mind filling all these out too, in addiiton to the eval from my home institution?" Especially the letters that were written for me a few months ago, before I knew where I was applying. How are you guys doing this? Am I making this more difficult than it's supposed to be???

Not to be a tool, but I think you just have to ask. I've had to do the same thing, it's a bit weird to do.

The way I look at it, I'm not the world's strongest applicant on paper (avg Step 1), so the least I should do is get all the paperwork in. Another way to look at it is that if 80% of the pool got everything in and you didn't...

Yeah, I knew the answer, I just didn't want to think about getting the forms to my AI letter writers. Oh well, I'd rather suck it up now than on un-match day...

Hey. My list (taken from department websites) pretty much checks out with what's above. I found a few discrepancies:

University of Pennsylvania: No college transcript
University of Iowa: No MCAT/SAT scores

Also, some other programs with supplemental requirements:

UC Irvine: CPR, ACLS, BLS certificate prior to interview date
Case Western: College transcript

If earlier posters have different sources of information for supplemental requirements, please feel free to correct me on the Iowa and Penn info.

Hope this helps. Not looking forward to asking for 4 to 5 supplemental forms to be filled out. Especially from letter writers at away institutions.

for the programs that say they want a CV, do they mean just the ERAS version of our CV, or do you think they want a hard copy of our regular CV sent to them?

a few programs I came across that list CV in their requirements......
Vanderbilt, Mayo, UPenn, Yale, Cincinnati

Though UPenn site does not say anything about a college transcript, the letter that they emailed me when I requested info says they do require a college transcript. So, looks like they do want the undergrad transcript.

Overall, the easiest way to find application requirements is to check the programs' websites. Doing a search on old posts is not accurate since program requirements may have changed for this year.

1. in Eras there is no choice of 6yr vs 5yr for the UMass program. Their application requirement website () does not state the requirement to indicate 6yr vs 5yr in the PS. Can someone provide some more advice?
2. In addition, UNC wants only TWO LORs ()
3. Iowa is not requesting MCAT and SAT scores but does want college transcripts ()
4. agree w/ ortho-pod on the UPenn college transcript requirement. I checked up their website a few months ago and the college transcript was a requrement. But that requirement has been recently deleted since I can no longer find it.
5. Could not find a supplemental form requirement for U Rochester ()
6. Could not find voluntary ID form for U Michigan ()
7. could not find Baylor's extra form for LOR requirement ()
8. The cleveland clinic does have an application form but this should not be required if you're submitting an ERAS application. I believe this is more for a Fellowship application. ()

Please, if anyone is posting application requiremets put down a reference. You can cause a lot of confusion by posting outdated materials.

I was just going to submit the ERAS application thinking that the ERAS CV is good enough for the "CV" requirement. I called a few programs (U Penn, CASE, Vandy, U Michigan) and the concensus is that ERAS CV is good enough. You can send a separate CV if it contains info that is not in the ERAS CV.

I talked to UPenn's coordinator today via phone. She told me that they no longer require an undergranduate transcript starting this year (which is why it is no longer on the website).

Forgot to include above, UPenn does not require an additional CV. The ERAS CV is what they want. This is from a phone conversation with the coordinator there.
